Where do I get Form 1095-A to file my taxes?

Form 1095-A (the Health Insurance Marketplace Statement) is mailed to you by January 31st each year and is also available to download directly within your online marketplace profile.

You must use this form to fill out IRS Form 8962 when filing your federal taxes. This process reconciles the tax credit subsidies paid to your insurance company throughout the prior year.
